In order to facilitate the scheduling process, we will need the following information:

* Legal name

* Date of birth

* Insurance information (Company, policy and group number)

* Contact information where you can be reached during the day

* Email address

* Credit Card

If you have had surgery by another Orthopedic Surgeon for the same problem in the last 6 months and are seeking a second opinion AFTER surgery has already been performed, please obtain all office notes, surgical reports, imaging studies (actual images and reports) and arthroscopic photographs (if applicable) that apply to the specific area you are requesting to have evaluated. These will need to be submitted for Dr. Bailie’s review PRIOR to being able to schedule an appointment. There are many reasons for this and should you require further explanation, please feel free to email him directly at dsb@AZISKS.com.

If you are requesting an appointment for a second opinion on a knee or shoulder that has NOT had surgery in the last 6 months, the records do not need to be submitted prior to scheduling an appointment. Please bring all operative reports, imaging studies (images and reports) and arthroscopic photographs (if applicable) to the appointment. Prior physician clinic records are often not required for the initial consultation, but may be needed if questions exist after your appointment.

If you are an existing patient and registered with the patient portal, you may REQUEST an appointment time electronically by clicking SCHEDULE APPOINTMENT. This request process will send a notice to Patricia that you are requesting a specific appointment date/time. She will check for any conflicts in our schedule and either confirm the appointment as requested or contact you to assist you with any changes.

You will also receive an email reminder for your appointment from appointments@email.practicefusion.com and may complete the intake form within that email in lieu of most of the forms on our website. Completing all required paperwork ahead of time electronically will prevent delays in your appointment as you will not need to complete anything upon arrival at our office. Please add this email to you contacts to avoid it arriving in JUNK or SPAM folders.
